Super 8, 5246 Harding Ave.: June 27: Three violations: Test kit not available for checking sanitizer concentrations; drain line for the last sink disconnected and not in good repair; wall coving missing around door frame in kitchen area.

Micheles Restaurant & Catering, 513 Division St.: June 27: Three violations: Food establishment does not have a required plan for reduced oxygen packaging cooked foods; eggs obtained from an unapproved source; no irreversible registering thermometer for measuring utensil surface temperatures in the hot water dish machine.
MTB-The Bar, 1396 Portage County C: June 6: Five violations: Test kits not available for checking sanitizer concentration; eggs obtained from unapproved source; cooler/freezer not certified or approved by department; cavity or door seals of microwave soiled; a few areas of kitchen floor in disrepair.
Muse at Sentry, 1800 N. Point Drive: June 11: Four violations: Foods held in reach-in prep cooler are not at proper temperature (corrected during inspection); several foods held in refrigeration units have exceeded their date marks (corrected during inspection); several foods held in refrigeration missing date marks (corrected during inspections); prep line reach-in cooler not maintained in good repair.
Papa John's Pizza, 101 N. Division St.: June 25: One violation: No certified food manager for establishment.
Sentry Division Street Cafe, 1501 N. Point Drive: June 4: Two violations: Cream cheese packets stored out of refrigeration at self-service area (corrected during inspection); sliced and diced tomatoes in prep coolers have exceeded date marks (corrected during inspection).
SentryWorld Center, 601 N. Michigan Ave.: June 13: 10 violations: Sliced tomatoes and lettuce held at refreshment station not at proper temperature (corrected during inspection); cherry aioli dressing has exceeded date mark (corrected during inspection); test kit not available for checking sanitizer concentration at ware-washing sink; piercing tip of table mounted can opener dirty; interior surfaces of ice machine dirty; food labels missing from sandwiches and wraps made and packaged on premise and sold at refreshment stations; thermometer missing from reach-in pizza prep cooler and ice cream freezer; no irreversible registering thermometer provided for measuring surface temperatures in hot water dish machine; blender jar for blender in bar has cracked surfaces; handwashing sign missing from employee handwashing sinks at refreshment station No. 1 and No. 2.

Stevens Point Country Club, 1628 Country Club Drive: June 25: One violation: Upright cooler at pool food stand not at proper temperature.
Subway No. 112 - East side, 1201 Badger Ave.: June 12: One violation: Employee not washing hands after handling money and prior to putting on gloves to assemble sandwiches.
Yang's China Wok, 135 N. Division St.: No. 5: June 26: Four violations: Raw meats stored above ready-to-eat foods (corrected during inspection); cardboard used to line floor of walk-in freezer; clean plastic soda cups stored or stacked without being air dried; light fixture above freezer in kitchen not working (corrected during inspection).
